Supermarket Checkout Personal Injury

A young customer was able to gain access to the underside of a checkout conveyor. The conveyor should have stopped but it did not. The child's arm was trapped in the conveyor mechanism. Fortunately the child escaped with only minor injuries.

In addition to establishing the cause of the accident the investigation involved consideration of the roles and responsibilities of many different contractors employed during modification works prior to the accident.
